Health and Disability - Home base support services

  • Main service of the company.
  • 400 clients per year for people over 65 and under 65 for health authorities' contracts; birth to 80 plus for ACC regional contract.
  • Variable rates is charged for ACC clients to provide rehabilitation service.
  • Provision of community nursing is part of this service.
  • 85% of service users are non-Pacific.
  • Provides Personal Care and Household Management to Qualified clients under the Needs Assessment and Service Coordination Agency criteria.
  • PIHC LTD operates a sub-regional service to over 65yr olds and regional for under 65yrs.
  • Cover most forms of disabilities except Mental Illness.
  • Special programs designed for Intellectual Disability clients aged 5yrs-45yrs.

Cardiovascular Disease (CVD) Services - WDB Contract for 2 Years

  • Delivered through nursing outreach clinics every fortnight where on average 2 to 3 new clients attended; held 3 clinics in the churches and 1 in a community centre, and the latter have attendance averaged 15 to 20 people.
  • Identifying at risk people.
  • Lifestyle changes through the Health Education/Promotion service below.

Health Promotion & Health Education + Outreach Nursing Services to Improve Access

  • Targeting Pacific population only in North Shore.
  • Averaging 12 education sessions per month for Samoa, Tonga, Cook, and Niue and mixed groups.
Cultural Support Services - WDHB Contract
  • North Shore Hospital based.
  • Pilot of cultural support for Pacific patients and now under review.
  • Targets achieved exceeded contractual volume.
  • Language barrier cases; complex cases, e.g. where families are apprehensive about medical services; full-time service where 1 staff is present; church ministers - provided the extra responsiveness and comfort for the client.
Nutrition & Physical Activities
  • To improve health and well-being and to reduce the burden of diabetes, obesity and cardiovascular disease through regular physical activity and improved nutrition by increase levels of physical activity for inactive adults and improve nutrition for Pacific people.
Healthy Eating & Healthy Activity
